Health Visitor

The Health Visitor gives help, support and advice to parents of babies and pre-school children. A further role is to accept referrals from other members of the practice team for specific areas of help with elderly patients. The child development and immunisation clinic is held on a Thursday morning, 10 am - 12 midday, appointments are necessary. Home visits to children or elderly patients are made by prior arrangement. The Health Visitor is also closely involved with the programme of Annual Health assessments for those aged over 75 years. The telephone number for the Health Visitor is available from reception.

The Community Psychiatric Nurse Counsellor

The CPN sees patients on receipt of a referral from the practice and requires completion of the patient voluntary opt-in return form.The counselling service is available for a range of mental heath problems. The telephone number for the CPN is available from reception.

District Nurse

The District Nurse can visit you at home to provide nursing care. This is arranged by referral by a member of the practice team or may also be requested by other health care professionals involved in your care such as hospital ward or out-patient staff. Community Midwife

The Community Midwife leads the practice based ante-natal clinic offering maternity care to expectant mothers and is supported in this by the wider based specialist Community Midwifery Services. The routine midwife led ante-natal clinic is usually held every second Wednesday morning. The telephone number for the Community Midwife is available from reception.
